Job Scope 1. Primary Care Engagement & Partnership
Develop and implement strategies to expand the network of primary care and community partners, ensuring care within the community is accessible, appropriate, and affordable.
Build and sustain relationships with GPs to understand their strengths and needs, working together to meet the health and care needs of residents in Singapore.
Facilitate resource connections for primary care doctors and collaborate with stakeholders inside and outside to strengthen their capabilities in patient care and address system and process challenges.
Oversee the contact database and manage strategic alliances with primary care partners to ensure ongoing collaboration and partnership continuity.
2. Care Models & Service Planning
Assist in planning and implementing collaboration programs, including shared care initiatives, to seamlessly integrate primary and tertiary care, enhancing patient outcomes.
Support efforts to improve service access for primary care partners, including direct access, shared care, and community care services.
Participate in the development and implementation of quality improvement frameworks to align with Healthier SG\'s preventive care goals in collaboration with primary care providers.
Identify opportunities for capability enhancement and facilitate the use of collaboration platforms to share best practices, insights, and training.
3. Financial Management Support
Drive continuous process improvement and regularly review assigned programs and projects to align with organizational goals and meet the expectations of patients and the community.
Execute tasks as directed by the Supervisor or Director.
Requirements:
Bachelor\'s degree in a related discipline.
Minimum of 2-3 years of experience in community care, marketing, or business, ideally within a healthcare setting.
