Job Description


The Population Health Management (PHM) Directorate has been established to support daily clinical management to become more systematic and targeted; to ensure NHG is more informed by clinical, operational and demographic data as Regional Health Manager for Central and North Singapore; and that this informs the way in which we deploy our collective resources across all organisations to maximise the health outcomes of the local population, and establish system planning and care delivery so it is more coherent and systemic in its understanding of population health outcomes, value, and equity.Our role in the PHM Directorate is to facilitate this change across NHG at population, provider, place, and resident level - ensuring that we address not just the clinical and preventative aspects of care but also the social, environmental, and behavioral determinants of health - so that we can achieve our ambition to add 15M health years for our residents.This means that the Population Health Management and Value Directorate is principally an \'enabler\' of this operational and cultural change - developing enabling infrastructure and building new capability at every level of our system in NHG on behalf of our partners and population.Major Duties & ResponsibilitiesThe Senior Executive will support the Population Health Management (PHM) leadership in developing and maintaining comprehensive population health profiles, segment populations based on health data and risk factors, drive the population health research agenda, and track and analyse payment model projects indicators. Their contributions will be essential to improve care quality, outcomes, equity, and optimise system value for defined patient populations and NHG residents.The post holder will support the team in the following areas:

  • Drive population profiling and segmentation to inform system planning and care delivery to become more systematic and targeted, and
  • Establish the population health research agenda at the NHG level, and
  • Support the tracking and analysis of payment model projects for reporting and improvement
Population Profiling and Segmentation
  • Develop and maintain population segmentation framework for NHG PHC to enable matching of use cases to appropriate population segmentation tools
  • Apply population segmentation approaches to support interventions, such as but not limited to
  • Tools: Modified British Columbia Framework, Johns Hopkins Adjusted Clinical Groups System
  • Use Cases: NHGP Polyclinic Subscription Plan
  • Keep abreast of other clusters\' segmentation efforts and identify areas of synergies
Population Health Research Programme
  • Support the set up of the Population Health Research Programme including development of a proposal paper, engagement materials for partners and funders
  • Maintain a repository of population health research programmes in close collaboration with researchers, academics, and operations colleagues
  • Enable the sustainability of the programme by identifying potential funders, and maintain oversight of the grants as a grant management office administrator
Population Health Outcomes Monitoring
  • Develop, maintain and monitor NHG\'s performance for key PHM-driven projects
  • Track and report on key payment model project indicators
  • Support in the analysis and prediction of value-based performances of ongoing payment model projects to align stakeholders towards achieving targets
Job Requirements
  • Good degree with at least 3 years of working experience (including a supervisory role), OR
  • Medical, Allied Health or Nursing degree with at least 3 years of working experience
  • Master\'s Degree in Public health would be an advantage and can be used to substitute work experience
  • Relevant experience in population health, healthcare planning, product management or program management roles preferred
  • Working knowledge of Microsoft Office required and experience in data analytic and/or primary and secondary research would be an added advantage.
  • Strong written, analytical and communication skills;
  • Good people management skills and ability to manage multiple projects with effective time management;
  • Ability to work well independently and as a team player.
