Healthier SG Implementation Office (HSI) seeks to develop integrated programmes for implementation including clinical care protocols that enable GPs to deliver consistent quality care to residents enrolled in Healthier SG, operationalise the concepts by which primary care physicians can systematically leverage community resources and services, and implement enrolment of residents onto Healthier SG. HSI will also drive overall project management for the implementation of Healthier SG programmes, through approaches that integrate ops-policy-tech-comms efforts.

You will be part of the Public Enrolment & Community Partnerships workstream to plan for the implementation of ops-policy details of the resident enrolment and service journey, and develop and facilitate the implementation of social prescribing and use of community resources, through community partners and other self-help channels. This will support enrolled residents to adhere to treatment and sustain behavioural changes to improve their health outcomes.

Duties: o To assist with the delivery of the resident enrolment and service journey plan for the smooth onboarding of residents onto Healthier SG, in partnership with community partners in the healthcare landscape

o Prepare for public enrolment when Healthier SG launches:
  • Refine value proposition of Healthier SG in a way that would motivate residents to enrol
  • Enrolment plan for eligible residents, starting with the elderly chronic
  • Specific enrolment plan for seniors and \xe2\x80\x9cNon-Tech\xe2\x80\x9d residents
o Prepare Healthier SG Public Communications Plan and Materials:
  • Define key value proposition to \xe2\x80\x9cmarket\xe2\x80\x9d to GPs/Residents
  • Prepare collaterals, videos, advertisements
  • Support MOH-led Comms & Engagement efforts
  • Ensure timely communication of information to the public
o Establish community linkages (with AIC, HPB, PA etc.) that enable GPs/PCPs to leverage community services and resources to support residents/patients to adhere to behavioural changes that contribute to better outcomes:
  • Define scope and operational details for community partnerships
  • o Develop Community Partnerships and streamlining existing initiatives
  • Coordinate with HPB/AIC/PA/SportSG on the model of delivery
  • Define roles of various community partners, mode for referral, compliance tracking
  • Aggregating info on existing initiatives and streamlining to Healthier SG platforms
  • o Support IT & Data system access to services/initiatives provided by Community Partners
Job Requirements:
  • Degree in any discipline, but familiarity with programme management in health and community/social care would be preferred.
  • Strong skills in programme development and management, with at least 5 years of relevant experience
  • Good project management skills, with the ability to track multiple ongoing projects meticulously, and to meet tight deadlines while remaining flexible and adaptable.
  • Strong interpersonal skills, with the ability to engage, coordinate efforts and relationships across different stakeholders effectively.
  • Strong analytical mindset to resolve conflicts and ability to exercise critical thinking in mitigating potential risks in project areas.
  • Good understanding of community-based organisations and linkages with community partners.
  • Experience in the healthcare industry and previous training in public health are advantageous, but not required.
  • Position will commensurate with qualifications and relevant working experience.
