Job Description

The Social Work Assistant will be responsible for receiving and processing applications of admission into The Salvation Army Peacehaven Nursing Home. He/She will also be required to provide administrative support and coordination to the Social Work department on new admissions.
Key Responsibilities
- Coordinate and setting up weekly/monthly Multidisciplinary Team Meetings (MDM)
- Prepare documents for financial assistance applications
- Collate statistics and submit monthly reports
- Conduct and follow up in means testing
- Coordinate home assessments and visits where necessary
- Post discharge follow up
- Organise events
- Attend to enquiries
- Assist in admin work
- GCE ‘O’ Level/Diploma
- Experience in Social Work, especially in healthcare industry would be an added advantage
- On-the-Job Training will be provided
- Proficiency in Microsoft applications
- Highly organised, independent with good time management skills
- Good interpersonal skills, accountable and responsible attitude
- Possess positive working attitude and ability to work with minimal supervision
Working Hours
5 days per week, Monday to Friday
8am - 5.15pm or 8.30am - 5.45pm
