With a capacity of around 400 beds, Peacehaven Nursing Home provides compassionate, individualized, holistic care to all without discrimination. We offer a comprehensive gamut of services to care for the sick, frail and elderly who have physical or mental disabilities with minimal or no appropriate caregiver support at home. The Home has 14 separate Resident Living Areas (RLAs), each designed as an individual home-like environment. Hope RLA is specially designed to meet the needs of relatively independent residents with dementia. DESIGNATION : Social Work Assistant (PHNNH)

RESPONSIBILITIES

• Receive and process applications from AIC & CGH or other outside sources of referral.

• Provide administrative support and coordination on new admissions.

• Coordinate and setting up weekly/monthly Multidisciplinary Team meetings (MDM). To follow-up when required.

• Coordinate Family Care Conference and Client Care Conference.

• Conduct and follow up in means testing.

• Prepare documents for Medifund and any other financial assistance applications if necessary

• Coordinate home assessments and conduct home visits when necessary.

• Post discharge follow up

• Provide administrative support

QUALIFICATIONS

• O/A Levels, preferably with certificate of office skills
• Preferably with 2 years relevant working experience
• Have good communication and interpersonal skills and excellent telephone etiquette.
• Should be a team player who is patient, warm and helpful and organised at work
• Knowledge of software packages such as Microsoft Office and Excel is required

OTHER INFORMATION
Working Hours 44 working hours Under normal circumstances the hours of duty will be as follows: 8.30am to 5.45pm - Monday to Friday (45 minutes lunch break)
