Job Description

: Supports financial assessment and applications of financial schemes under supervision of medical social workers (MSWs). Assist in initial assessment and basic case management. Performs Information and Referrals function. Conducts : Supports financial assessment and applications of financial schemes under supervision of medical social workers (MSWs). Assist in initial assessment and basic case management. Performs Information and Referrals function. Conducts home visit independently or joint visit with MSWs or Home Care team where necessary. Supports the administrative functions of the department. Works closely with multi-disciplinary team members. Support projects and programme development. Attends training and case work supervision. Any other related duties as directed by the said reporting officers. Requirement Diploma in Social Work or equivalent Able to communicate with Mandarin-speaking patients.
