Lead the team to provide effective and efficient operational support to the clinical departments under the department\xe2\x80\x99s charge
Lead the operational development, implementation and mainstreaming of clinical programs under the Division of Integrated and Continuing Care together with Medical and Nursing leads.
Work with external and internal partners to develop and support care transition services and programs between the organization, primary care and the community care ecosystems.
Participate in the development of the integrated and transitional care strategy for the company
Requirement:
Recognised University Degree, preferably in health and social sciences or business degrees. A Master\xe2\x80\x99s Degree in Public Health, Epidemiology or equivalent will be advantageous
Minimum 8 years of relevant experience in healthcare, community, or social care sector. Experience in community and transitional care, programme development & delivery, training and partnerships will be advantageous
