Assistant Director, Clinical Ops (integrated Care)

Job Description
- Lead the team to provide effective and efficient operational support to the clinical departments under the department\xe2\x80\x99s charge
- Lead the operational development, implementation and mainstreaming of clinical programs under the Division of Integrated and Continuing Care together with Medical and Nursing leads.
- Work with external and internal partners to develop and support care transition services and programs between the organization, primary care and the community care ecosystems.
- Participate in the development of the integrated and transitional care strategy for the company
- Recognised University Degree, preferably in health and social sciences or business degrees. A Master\xe2\x80\x99s Degree in Public Health, Epidemiology or equivalent will be advantageous
- Minimum 8 years of relevant experience in healthcare, community, or social care sector. Experience in community and transitional care, programme development & delivery, training and partnerships will be advantageous
