Job Description


:

  • Collaborate with government agencies, hospitals, community service providers, internal and external healthcare teams to provide coordination and continuity of patient care across healthcare continuum
  • Conduct and participate in case conference
  • Act as a coordinator for admission of residents under Ministry of Health (MOH) Portable Subsidy
  • Perform financial counselling for all residents under MOH\xe2\x80\x99s Portable Subsidy, including the preparation of estimated bill size, admission contract and other administrative matters
  • To perform National Means Testing (NMTS) for portable subsidy residents and ensure the resident NMTS are valid and updated accordingly to MOH\xe2\x80\x99s requirement
  • Act as a resource person for the MOH ILTC portal for all ECON Medicare Centres and Nursing Home
  • Ensure timely submission by ECON Medicare Centre and Nursing Home of resident workload and as a result, ensure that the organisation receives the corresponding and correct funding from MOH
  • Assist the resident and their families to put up application for Nursing Home subsidy for ILTC Residential placement
  • Perform assessment on the resident and their families eligibility for financial assistance
  • Assist the resident and their families to put up application for the following:
  • Shelter Home
  • Day Care
  • Day Rehab
  • Home Modification
  • Senior Mobility Fund etc
  • Assist the resident and their families in putting the applications for government financial schemes. Applications would include the following, and other schemes that may be available
  • Interim Disability Assistance Programme for the Elderly (IDAPE)
  • Eldershield Claim
  • Medical Fee Exemption Card (MFEC)
  • Medifund Claim
  • PGDAS etc
  • Ensure all centres submissions to the ILTC portal meets the MOH Financial audit requirements
  • Perform such other duties and responsibilities that may be assigned from time to time
  • Assist the Head of Department (HOD) in providing supervision guidance and precepting assigned staff to ensure works are appropriate quality and standard
  • Assist HOD in assessing staff competency and identify learning needs
  • Assist HOD in reviewing and update policies and procedures to meet ministry regulatory requirement
  • Assist HOD in reviewing and develop workflows to ensure seamless admission of residents.
REQUIREMENTS
  • Degree in Social work or any equivalent qualification with at least 3 year relevant working experience
  • Good interpersonal and communication skills
  • Team player
  • Service oriented and customer-focused
  • Competent in IT skills
  • Ability to work under pressure and meet tight deadline
