COMPANY DESCRIPTION NTUC Health Co-operative Limited (NTUC Health) is an NTUC social enterprise that provides a comprehensive and integrated suite of quality and affordable health and eldercare services to meet the growing needs of families and their dependents. Building on more than four decades of experience and expertise, NTUC Health is among the largest senior day care, nursing home and home personal care providers in Singapore. It also offers other services for seniors such as an active ageing hub, senior activity centres, community support for vulnerable seniors, and a sheltered / senior group home. In addition, it runs a chain of dental clinics, and a family medicine clinic RESPONSIBILITIES To provide basic care coordination, discharge planning, monitor/evaluate and education for clients and their families on the services offered by NTUC Health Co-Operative Ltd. Information and Referral (I & R)

• To manage various referral sources such as emails, phone calls and walk-ins in order to link clients to appropriate services by identifying and accessing their needs.

Service Coordination

• To administer the coordination of service requested by new clients.
• To ensure proper documentation on the systems and paper filing are completed on a timely basis.

Follow-up

• To conduct regular telephonic follow up to engage with clients and obtain feedback on the services rendered. To arrange for reassessment of care plans with clients on a 6-monthly basis. This is to determine if client requires to continue to receive the service
• To work closely with Operations and Nurse Manager on discharging clients from service

QUALIFICATIONS

• Diploma in business administration, healthcare related disciplines
• Good interpersonal and writing skills
• Good team player and multi-tasker
• Be proficient in English and Mandarin
• Be proficient in computer applications such Microsoft Office
• Good listener, outgoing and meticulous
• Able to adapt in a fast-paced environment

OTHER INFORMATION

• Possess at least 2 years of customer service experience
• Experience working in Community/Eldercare/Healthcare Sector
• Possess passion in serving the community
