To co-ordinate care for patients and families in a community-based preventive paediatrics health programme between community sites (preschools), social workers and healthcare professionals from the Child Development Unit.
To manage patients on a health preventive programme with screening and health education.
To guide and support patients and families in building healthy habits by providing education, motivation, and accountability through in-person and/or teleconference sessions.
To participate in management of clinical data collection, including database management using RedCAP, excel, or provide surveys through designing platforms for data collection with team
Duties and Responsibilities
Assess and identify children and families\' medical, psychological, social, environmental, and financial needs through simple screening questionnaires.
Provide administration of child health, developmental and family needs screening questionnaire i.e. ESQ, PHQ-9 in a community-based setting including home visits, school visits and via teleconference.
Direct patients and families into hospital or partner programs as per the protocol.
Collaboration with clinicians, nurses, other healthcare members, and patient/family to implement, monitor and evaluate care plans and follow-up on outcomes.
Provide health coaching to patients and families on different topics with the aim of building healthy habits i.e. screen time, nutrition, sleep and etc.
Conduct follow-up visits and phone calls to provide support and track progress after the introduction of health coaching and other programme initiatives at designated time points.
Support necessary administration such as documentation, data collection and correspondences.
Perform other duties related to the implementation and evaluation of the program as assigned by reporting office/ programme in-charge.
Job Specific RequirementsQualification and skills
A bachelor\'s degree in relevant disciplines (e.g. Nursing, Psychology, Behavioural Science, Social work, Health Sciences, Education)
Keen interest in early child development.
Excellent command of spoken and written English
Strong interpersonal and communication skills. Patient and personable.
A team player with an ability to work independently.
Experience
Experience of 1-2 years in hospital settings or social service sector work.
Experience working with preschool-age children and/or families is an asset for this position.
