Job Description

STATEMENT OF PURPOSE The case Manager supports and manages the patient’s overall recovery and works closely with clinicians in providing appropriate services and treatment. The Case Manager plays an integral role in the multi-disciplinary team; they act as a bridge between patients and psychiatrists, also helping patients transit back to their community and family setting. MAJOR DUTIES AND RESPONSIBILITIES (A) SPECIFIC (80%)

• Develop a partnership with patients, assess and anticipate care requirements, make plans and ensure that patients receive appropriate care and services.
• Achieve anticipated clinical outcomes and involve the care team by providing information and updates on patients’ treatment progress.
• Advise Manager and Program Director about issues relating to patient safety.
• Develop and improve departmental systems.
• Adhere to professional standards and integrity as outlined by protocols, rules and regulations.
• Document all case information, forms and reports accurately
• Support the department in development and goal achievement

• Promote the importance of Mental wellness in the hospital and community.
• Develop a strong cohesive team within department and hospital.
• Promote a patient- orientated culture that emphasizes in quality, continuous improvement and efficient services.
• Enhance overall hospital services and development.

• Degree or equivalent experience – Degree in Psychology, Social work or Nursing
• Years of Experience: 1-2 years in a mental health, social service, hospital or healthcare environment.
