Overview and purpose of the position
The incumbent will assist the Director and Assistant Director, and work closely with internal and external partners and stakeholders to implement and support Population Health initiatives in primary care.
Responsibilities
1. Implement Population Health initiatives in primary care
2. Implement evaluation, performance and reward activities
3. Administer and manage the Population Health related finances and audit
4. Engage and form collaborative relationship with internal and external partners and stakeholders as required
5. Provide secretariat support for key committees and workgroups, and gather insights/feedback from partners for the planning and implementation of Population Health initiatives
6. Any other tasks as assigned
Qualification Required and Area of Discipline
Bachelor degree preferably in health and social related fields
Required Competencies and Capabilities (Skills, Experiences and Professional Licences)
