Purpose of the Role

• Build partnership with the community stakeholders, patients and care-givers to assess needs, facilitate care integration and improve the health of the population.
• Work with stakeholders to coordinate, plan, organise and execute health promotion activities in the community
• Analyse results, document and communicate the outcomes of such interventions

• Contribute to the administrative tasks of the office.

• Bachelor / Master degree preferably in Public Health, Social Work, Health Sciences or Communications.
• Ability to communicate well, verbally and in written form
• Highly dependable, organized and effective collaborator
• Managing multiple projects simultaneously and attentive to details
• Able to handle detail oriented work while meeting schedule and timelines
